＊Notice!; An appointment must be made in advance with a referral from primary doctor to consult for Orthopedics, Otorhinolaryngology, Neuropsychiatry, and Radiation therapy.
（整形外科、耳鼻いんこう科、神経精神科、放射線治療科は、かかりつけ医からの紹介による事前予約が必要です。）

- ＊Notice!；If you do not have a referral letter, you need to pay 7,700 yen (tax inclusive) as an additional fee to the first visit（紹介状がない場合、選定療養費がかかります。 初診時：7,700円）

If you do not have a Japanese health insurance card or credit card, you will be asked to pay 80,000 yen, which is the approximate amount of your medical expenses, before the examination.
（日本の健康保険証または、クレジットカードを持ちでない場合、診察前に医療費の概算にあたる金額(8万円)を支払っていただきます。）
After the examination, we will calculate the medical expenses and refund you the difference or ask you to pay additionally.
入院が決定した場合は入院時に推定される医療費に応じたデポジットを支払っていただき、退院時に医療費を計算して差額を返金または追加で支払っていただきます。

海外旅行保険やお住まいの国の 医療保険を持ちの場合、 全額自費でお支払いいただき、ご自身で各 保険会社への請求手続きを行ってください。当院では保険会社と直接交渉はいたしません。
